Dean's Certification
Obtaining a Dean's Certificate for graduate or professional school or as part of an employment or armed forces investigation:
Complete the personal information section sent by the institution to which you are applying.
If the institution does not provide a release of information form complete a FERPA release of student discipline information form and send it to the address below.
The certification will be mailed directly to the admitting/inquiring institution.
A Dean's Certification only releases student discipline information, for transcripts or grade releases contact the registrar's office.
Response time is aproximately 5 business days.
