Immunizations
We encourage all students to be current on their immunizations. We do not currently have immunization requirements other than mandatory tuberculosis (TB) skin testing for students born outside the United States. This is a pre-matriculation (pre-enrollment) requirement.
Adobe Reader is required to view any of the following forms.
Mandatory TB Test
- Required for all persons born outside the United States.
- We do not accept copies, faxes, scanned copies or emailed TB test forms, must be original TB documentation form.
- Students must have had the TB Test within one year of registration.

- We do not accept x-ray film. Please do not send x-ray film.
- A TB test must be read within 48 to 72 hours or it will not be accepted for approval.
- A TB hold will remain on the student's record until the testing has been satisfactorily completed and the TB form has been returned to the Student Health Center and approved. Students are required to have the TB test regardless of status of application.
We are unable to confirm receipt of TB testing documentation form by fax or email.
